How do I access the online medical report? 
You can go directly to the online medical report by typing the following into your web browser:  medicalreport.vicroads.vic.gov.au  For easy access to the online medical report, save the link as a favourite on your web browser

What devices can I use to access the Online Medical Report?
The online report can be accessed on most computer desktops and laptops (Windows and Mac OS). At present the form is not supported on mobile or tablet devices.

What browsers can I use?
By using an up-to-date browser such as Google Chrome, Microsoft Edge, Safari or Mozilla Firefox, you are giving yourself the best chance to ensure your experience is smooth, secure, and error-free.

What is my AHPRA authentication number?
Your AHPRA registration number includes 3 letters and 10 numbers, and should be no more than 13 characters (ie. XXX8888888888) 

How do I save a draft in the system?
There is a button at the end of each page on the form that can be clicked to save your draft. You can find your saved drafts on your home page. Saved drafts expire after 60 days.

Can I save and print a final copy of the report?
Yes.  You can download, save and/or print a PDF copy of the report for your records after you have clicked “Submit assessment”. 

Alternatively, you can access any reports that you have submitted in the last two years at any time by logging on to your home page (after entering AHPRA number and DOB).

It’s important that the reference number (located on the ‘Assessment complete’ page or the PDF copy of the report) be provided to your patient as this is their reassurance that they have fulfilled their obligations and can be used for any follow up with VicRoads. 

How do I know VicRoads has received the completed (and submitted) report?
When the form is completed and you press the “Submit assessment” button, you’ll see a reference number and a link to a pdf of the assessment report for you to save or print. Once the "submit" button has been pressed the report is automatically received by VicRoads. The assessment report pdf will include a report reference number, which you or your patient can quote to help make enquiries with VicRoads.

It’s important that the reference number be provided to your patient as this is their reassurance that they have fulfilled their obligations and can be used for any follow up with VicRoads. 

What security practices are in place to ensure my personal details and my patient’s details are protected in the system?
When you complete an Online Medical Report for your patient, you can rest assured that their medical assessment details, including all personal and health information you provide on the report will be kept secure and confidential as per VicRoads usual processes as outlined in the Road Safety Act 1986. This also applies to your AHPRA number. Your date of birth information is required for AHPRA confirmation and is not retained by our system.

Will the Online Medical Report be included in prescribing software?
No. The most accessible means of using the report is via a direct URL link from VicRoads website. We have considered privacy, confidentiality and security issues in the design of the Online Medical Report.

Will information received from health professionals via the Online Medical Report be automatically linked with My Health Record?
VicRoads does not share information with the My Heath Record. You and your patient can decide whether you wish to upload a copy of the Online Medical Report to the patient’s My Health Record.

Can I still use the paper form?
Yes, however the paper form will be revised to align with the national medical standards for drivers (AFTD guidelines) and the online medical form.  As a result, the paper form will change and be updated on the VicRoads webpage at a later date. Note that if you mail the form to VicRoads you need to allow up to two weeks for delivery.
